QUALITY OF LIFE(NEXT GENERATION) HIP SURGERIES , Bangalore , India - UNCEMENTED MODULAR BIPOLAR HIP ARTHROPLASTY IN 84 YR OLD WITH PARKINSON S DISEASE

With the increased longevity and increasing geriatric population , hip fractures have increased in numbers and with the present generation of implants , surgeons  are able to provide excellent quality of life hip surgeries hitherto unavailable just a decade ago.We at orthoone orthopedic and joint replacement center are able to provide surgeries for patients with hip fractures, hip arthritis and redo hip surgeries  options which gives them an excellent quality of life and good results in patients with advanced age and multiple health problems (diabetes, HT , advanced age, neuro problems and liver cardiac and kidney diseases ).

The older concepts of cementing has been turned on its head by the present generation of highly refined and highly indivisualized / modular hip implant .
they provide superior fit minimalizing the risks , tissue trauma  and time in cemented implant.

POST OPERATIVE X RAY

WHAT  AND WHY WE DID
we chose an uncemented component due to the excellent stems now available and minimal morbidity to patient , less operative time , less tissue trauma  and to lessen cardiac complications in very old patients where surgery is a stress and risky.

we performed a right sided iliopsoas and adductor release(bilateral) and got very good correction of contractures and his hips which were flexed and adducted prooperatively were abducted and he can now walk comfortably with walker.

POST SURGERY -PATIENT STANDING ON DAY 1- walked few steps with walker

 REVIEW 2-

I was at the height of physical fitness, trying to do everything possible to keep myself fit. On 11th July 2009 I had planned to trek with few of my friends at Bilikallu Rangaswamy betta near kanakapura town. After traveling through very serene and winding roads, we had to trek for about 3.5 hours to get to the top of the mountain. It was very scenic and the winds at the top were very ...refreshing and we really enjoyed our time there. We started our descent and after 10 minutes I slipped and just had to squat. In the process, because of the momentum my right leg entered under a rock. When I pulled it out I realized that my foot had turned around by 90 degrees. When I tried putting my foot down it was clear that my ankle was completely dislocated. I was obviously in severe pain and my friends then had to help me get back to the vehicle. It took me 5.5 hours after the accident to get to the hospital. Orthopedic surgeon then asked for an X-ray and made it very clear that there were multiple fractures and a surgery had to be performed. My dad and uncle then wanted to take a second opinion on the same. They were then referred to Dr. Deepak Inamdar by a close family friend who is a physiotherapist. My dad and Uncle were obviously very confident once they spoke to Dr. Deepak Inamdar and wanted to me to be treated by him. I was then moved to Ramakrishna hospital. I met him for the first time at the hospital and he immediately infused lot of confidence in me. He was a young doctor who was willing to spend time to explain all aspects of what had happened and what he was planning to do. I finally had to go through the surgery on 14th July 2009 and it was a 3.5 hours surgery. He met me next morning in the ICU with the X-Ray of my fibula and ankle with the plate and screws fixed in it. He then explained me why it so long as the bone had shattered and it was a jigsaw puzzle for him to put that back together. He obviously took pride in the work he had done the previous day and was very thrilled that it had come out very well. It took me 4 months to move from ICU to ward to bed to crutches to stick and finally to walking. People were really imagined that was back on my legs and could start walking in a period of 4 months. In about 5 months I could resume walking for 2.5 hours continuously. In a period of about 8 months or so I was about play every game that I used to play earlier. In fact I had also gone trekking within a year to Sawan Durga although it was not a treacherous!! Real testimony to Dr. Deepak’s skill was when I had visited US in winter within 8 months of my accident and enthusiastically ended up running on the treadmill. I was in pain and had to visit a local doctor there and he was amazed at the work done on my leg and ended up saying that it he could barely make out that a surgery was done on my leg as it was really clean. Now I am able to do any activity without any hindrance at all, Thanks to Dr Deepak

Review NO 3-

I hardly have pain any more and my hand looks much better.

Were you pleased with the treatment? I had a radial club hand since by birth 30 years ago and none of the doctors i consulted, including a specialist in the UK, diagnosed it correctly. As a result i underwent painful procedures which only resulted in a deformed left hand and chronic pain in the wrist. A chance visit to Dr.Deepak revealed the problem and he suggested a cosmetic correction. The procedure was simple and results are amazing. I hardly have pain any more and my hand looks much better. Thank you doctor.

OSTEOARTHRITIS - MYTHS/ FICTION AND FACTS

Osteoarthrosis or osteoarthritis  basically means wear and tear of the cartilage, which can be divided into primary (due to the normal ageing process - happens to almost every one universally ) and secondary - due to a preexisting cause, disease , trauma or condition from birth.
one must understand that primary osteoarthritis is more prevalent in knee joints in asian population ,due to the habits and in the west a higher prevalance of osteoarthrosis of hips compared to asian population.

osteoarthritis affecting the knee joint -  is universal and part of ageing process , cartilage is a tissue with a very low  metabolic activity and hence cant be regenerated at a pace which can outrun its wear.
knee as a joint is abnormally shaped  unlike the ball and socket of the hip  it is  primarily designed to bear weight and provided stability  but makes it vulnerable for wear.
the wear happens more on the inner aspect of knee and later cases affects whole of knee joint resulting in pain in the whole of knee .

Therapies which work only in very early osteoarthritis of knee are -  arthroscopic debridement ,cartilage regenerating medication(dyacerin , glucosamines), viscosupplementation with hyaluronic injections , physiotherapy and analgesics.

but one should know these things may prolong and help an indivisual be pain free for a maximum of  few years and if lucky with activity modifications a few more years .

Treatment for advanced osteoarthritis-
High tibial osteotomy, uniknee replacement and Total knee replacement -is still the gold standard and only available option which has stood the test of time and which stands to reason for patients with advanced arthritis.

Questionable therapies exist widely all around the world which do not hold water do not stand to reason  for the simple  fact that - no one can reverse ageing / a worn out cartilage , despite what ever the advance in science . knee replacements work for the fact that they replace the worn cartilage hence give a relief of pain.
